Advanced Lipid Panels (ApoB)
Standard cholesterol tests measure the total cholesterol carried in LDL particles (LDL-C). However, atherosclerotic risk is driven by the *number* of atherogenic particles, not just the cholesterol mass within them. Apolipoprotein B (ApoB) counts every atherogenic particle directly.
Why ApoB is Superior
One ApoB protein sits on the surface of every VLDL, IDL, LDL, and Lp(a) particle. If your LDL-C is normal but your ApoB is high, you have many small, dense particles—a highly atherogenic state.
| Marker | Standard Goal | Aggressive Longevity Target |
|---|---|---|
| ApoB | < 90 mg/dL | < 60 mg/dL |
| Lp(a) | < 30 mg/dL | < 10 mg/dL (Genetically determined) |
Common Mistakes
- Never Testing Lp(a): Lipoprotein(a) is highly atherogenic and genetically driven. Diet won't fix it. It must be checked at least once in a lifetime.
Frequently Asked Questions
Q: Does dietary saturated fat raise ApoB?
A: In most individuals, yes, by downregulating LDL receptors in the liver.
